Disability Accommadations
The Accommodation Application at Faith Theological Seminary of Catonsville is designed to support the review and approval of student disability accommodation requests. Through this process, students with documented disabilities may request appropriate academic accommodations and related support services.
If this is your first time submitting an accommodation request (as a new or current student), please complete the application form and have your disability documentation ready to upload. Examples of acceptable documentation include:
- Psychoeducational evaluation
- A signed letter on official letterhead from a licensed healthcare provider who has personally evaluated or treated the student and identifies a disability (documentation may not be provided by a family member)
